By registering my child for Heart Soccer Academy, I acknowledge and agree to the following:

1. Assumption of Risk
I understand that soccer training and physical activity involve inherent risks, including but not limited to falls, collisions, muscle strains, sprains, fractures, and other injuries. I voluntarily accept these risks on behalf of myself and my child.

2. Health & Fitness
I confirm that my child is physically able to participate in the program. I agree to inform Heart Soccer Academy of any medical condition, injury, allergy, limitation, or other information that may affect my child's safe participation.

3. Emergency Medical Care
If an emergency occurs and I cannot be reached, I authorize Heart Soccer Academy staff to seek appropriate emergency medical assistance for my child. I understand that I am responsible for any resulting medical or emergency expenses.

4. Safe Participation
I understand that my child is expected to follow coaching instructions, safety rules, and facility guidelines. Heart Soccer Academy reserves the right to remove a participant from a session if their behaviour or condition creates a safety concern.

5. Release & Waiver
To the extent permitted by law, I release and hold harmless Heart Soccer Academy, its coaches, staff, volunteers, and facility partners from claims arising from my child's participation, except where liability cannot legally be excluded.

6. Parent/Guardian Consent
I confirm that I am the parent or legal guardian of the participant and have the authority to provide this consent.
